Covered entities must prove from their own records that the prescription came from an eligible patient encounter within the entity’s responsibility for care. Referral orders, telehealth visits, refills, and contract pharmacy claims are not enough by themselves unless the chart clearly ties the prescription to an ongoing, documented care relationship.
