HRSA’s 340B patient definition requires records showing the individual received outpatient care from the covered entity, through an employed or arranged provider for whom the entity retains responsibility, and that the prescription arose from that care. A prescription alone or general patient status does not prove eligibility; the chart must document the relationship and outpatient drug status.
